POSITION TITLE

Case Manager/Program Coordinator

REPORTING TO

Detention Manager

POSITION SUMMARY

Position works in collaboration with the Juvenile Detention Management team to develop and implement programmatic case management plans for youth placed in the NORCOR Juvenile facility. This is responsible for service delivery of cognitive based interventions for youth. This position coordinates information sharing, services and planning within the facility, external youth serving agencies, and the Oregon Youth Authority.

DUTIES AND RESPONSIBILITIES

  • Responsible for the coordination and implementation of cognitive based interventions for the Youth Care Center and 30-day extended detention programs within the NORCOR juvenile facility.
  • Become a team member of the NORCOR Juvenile Detention facility, assisting detention officers with duties as required by the Detention Manager.
  • Review program referrals, manage and lead reviews with stakeholders, and coordinate services identified in case plans.
  • Documentation of activities, groups, and interventions in the Oregon Juvenile Justice Information System and communicate regularly with juvenile department staff and Oregon Youth Authority Juvenile Probation and Parole Officers (JPPOs).
  • Complete Risk/Needs assessments and screening instruments when indicated.
  • Deliver direct services, case management and group intervention within the juvenile detention facility.
  • Provide case management services that include the development and implementation of reentry and transition planning utilizing community providers and services.
  • Develop and implement incentives/practices that encourage the youth offender population to participate in indicated programming.
  • Responsible for forming partnerships with key community stakeholders where appropriate that aid in the reentry and transition the youth into their next placement.
  • Develop strategies that allow for Community in-reach of those providers and services that aid in the reentry process.
  • Engage juvenile detention staff in programming and problem-solving with youth to create a positive custody environment.
  • Other duties as assigned.

Qualifications

  • Bachelor’s degree in Psychology, Sociology, Criminology, the Behavioral Sciences or related field from an accredited college or university.
  • Minimum of 2 years of experience in working with youth involved in the juvenile justice or system involved youth who have behaviorally challenging attributes. Specific experience in areas of leading treatment groups and individual supervision. Experience with current best practices/promising programming is preferred.

PHYSICAL DEMANDS:

The physical demands described here are representative of those that must be met by an employee to successfully, perform the essential functions of this position. Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.

Work is generally performed indoors in a secure juvenile detention facility and is essentially sedentary in nature. The work requirements include the ability to see, talk and hear, sit and stand, and use hands to control tools; reach with hands and arms and lift or move (50) fifty pounds. Some work assignments directly involve the safety and security of the facility and may require immediate decision making. The facility operates 24 hours a day and hours of work may include days, evenings, nights, weekends and holidays.

WORK ENVIRONMENT

Detention facilities are unique by nature. Communications may be by electronic means, requiring visual and audio queues and codes. The noise level in the work place is usually moderate but is unpredictable, and can escalate to a loud level of abusive verbal sounds and terminology. There may also be occasional occurrences of speaker systems, percussion and abrasive sounds, alarms and recreational sounds.

There may also be the presence of various potentially contagious bacteria or viruses as well as the possibility of blood borne pathogens. The level of security is usually secured, but there could be situations where the safety of the employee is compromised, employees could be exposed to threats, and abusive language toward self, family and co-workers.

REPORTING TO

Mental Health Professional II and Jail Commander

POSITION SUMMARY   

Under direction, to provide comprehensive mental health treatment as part of a multi-disciplinary team; assess family, cultural, social and work relationships; conduct mental status examinations; identify current DSM diagnoses; write and supervise the implementation of an Offender Management Plans (OMP); provide individual, family and/or group therapy within scope of training; and, other related work as required.

This is the entry/journey level in the Qualified Mental Health Professional series. Incumbents perform a full range of duties with only occasional instruction or assistance. Responsibilities cover a full range of complex professional tasks. Employees are expected to work in an independent and professional manner and possess a significant level of specialized and functional expertise. This classification requires that employee exercise independent judgment in performance of duties and work within broad procedural framework. Employees may review the work of other employees or subcontractors.

DUTIES AND RESPONSIBILITIES

  • Completes comprehensive mental health assessment.
  • Develops and/or coordinate client Offender Management Plans; as appropriate, makes referrals to other services or community agencies to meet client need.
  • Provides specialized individual, family and/or group counseling and therapy, education, outreach and support to clients and families as necessary.
  • Provides consultation, education, information and resource advocacy to agencies in the community which routinely come in contact with clients; maintains liaison with key community agencies that serve people with mental illness.
  • Prepares and maintains required records, progress notes and files; prepares reports as needed
  • Assists with program planning activities, designing program services and coordinating activities with assigned staff.
  • Maintains knowledge of current research, trends and practices applicable to assigned program by attending classes, conferences and workshops as appropriate.
  • Serves on committees; performs other duties and responsibilities as assigned.
  • Adheres to County and departmental policies and procedures as well as safe work practices, policies and procedures.
  • Develops and maintains effective working relationships with other staff, public officials, the general public and representatives of other agencies.
  • Attends and participates in required trainings.
  • Has regular and reliable attendance.
  • Performs other related duties as assigned.

Maintains knowledge of current research, trends and practices applicable to assigned program by attending classes, conferences and workshops as appropriate

Qualifications             

MINIMUM QUALIFICATIONS 

Requires knowledge of the techniques and procedures used in evaluating, modifying and motivating human behavior; case management methods; rules, laws, regulations and ethics of mental health services and treatment; and community services and resources. Ability to assess status of individuals and families for services and needs; write concise and accurate progress notes and reports; coordinate treatment plan; provide individual and group counseling; maintain confidentiality; participate as an active member of a cohesive work groups; work independently and as a team member; exercise initiative, tact and good judgment in dealing with people from various backgrounds; provide support and guidance to clients and family members; handle multi-task situations well; maintain records with accuracy and confidentiality; communicate effectively, both orally and in writing; establish and maintain cooperative working relationships with those contacted in the course of work; communicate effectively and sensitively with patients; respond professionally and responsibly in crisis, emergency or dangerous situations.

EDUCATION    

A Master’s Degree in psychology, social work, or related behavioral science field

Employee must have a valid Oregon driver’s license with an acceptable driving record. And maintain License as a clinician in Oregon. Must currently possess an LCSW or obtain within 6 months from date of hire.

PHYSICAL DEMANDS:

Ability to work in an office environment; to coordinate hands and feet in performing simple movements, such as bending, reaching and grasping; to exert physical effort in sedentary to light work involving moving from one area of the facility to another, reaching, kneeling, climbing of stairs and standing for extended periods of time. Requires sufficient strength and coordination for lifting, pushing, pulling and/or carrying the weight of equipment as well as sufficient hand/eye coordination to perform semi-skilled repetitive movements such as operating office equipment. Requires the ability to operate a motor vehicle. Sensory requirements include sound, odor and visual perception and discrimination, as well as oral communications ability.
